The Kenya Health Surveillance Form is an online health declaration that all travelers to Kenya must complete before departure.
 
Travelers wishing to travel to Kenya must provide detailed information regarding their planned trip as well as basic personal information and answer certain health-related questions.
 
Completing the form takes a few minutes and can be done completely online using your phone, laptop, PC or tablet.
 
Kenya has introduced health surveillance to ensure the safety of Kenyan citizens and residents as well as all foreign visitors.

As long as your transit in Kenya does not exceed 48 hours and you do not leave the designated airport transit area, you are not required to fill out a pre-travel health monitoring form. However, online health declaration is mandatory for all longer transit stays or for those planning to leave the airport transit area during the transit period.

Kenya requires most visitors to take a Covid-19 PCR test within 96 hours of arrival to ensure their visit is safe. Additionally, Kenyan authorities have made it mandatory for all incoming visitors to complete an online health declaration before arrival. When arriving in Kenya, visitors will not face many restrictions as the mandatory curfew has been abolished.
