Children must have their own health monitoring form to enter Kenya. Parents or legal guardians of minor travelers must ensure that all data provided in their child’s registration form is accurate. Parents cannot include their children on their own forms.
To receive the online Kenya Health Surveillance Form, travelers just need to take a few minutes to fill out the online form with the required data, submit and pay the service fee using any online payment method any.
A unique QR code is issued and sent to each traveler who applies for a Kenya Travel Health Certificate online. This code will reach each traveler via email and must be printed before entering Kenya.
To apply for a Kenya Travel Health Certificate (Health Surveillance Form), you will need a valid passport, details of your planned travel to Kenya (e.g. flight and seat number), negative PCR test result and active email address.
