Sri Lanka Health Declaration is that online travel registration to Sri Lanka is now mandatory for all arriving visitors.
 
All visitors planning to enter Sri Lanka must complete the online health declaration by providing the requested information and attaching the necessary supporting documents within 48 hours.
 
The Government of Sri Lanka has introduced a requirement to complete pre-departure health declarations to ensure public safety during the ongoing Covid-19 pandemic.
 
For travelers under 15 years of age, the application must be completed and submitted by a parent or legal guardian.
 
Sri Lanka Health Declaration can be completed online by providing the required data in the application form using a phone, laptop, PC or other device with an Internet connection.
 
After completing the Sri Lanka declaration, travelers will receive a PDF confirmation via email with their unique QR code.

Requirements to complete the Sri Lanka Health Declaration Form

To complete the Sri Lanka Health Declaration Form, travelers must meet a few basic requirements, including having:

  1. a stable internet connection

  2. an active email address

  3. flight to Sri Lanka details

  4. a valid passport

  5. details of the planned visit to Sri Lanka, including the address for the stay

How to apply for the Sri Lanka Health Declaration

Travelers wishing to visit Sri Lanka must fill out the Sri Lanka Health Declaration Form online by filling in the application form with the following data:
  1. Personal Details - name, date of birth, gender, nationality, passport number

  2. Contact Details - email address and phone number

  3. Travel Details - address for the stay in Sri Lanka, country of departure, date of departure

  4. Flight Details - flight and seat number

  5. Health Status - answer some questions related to your health and Covid-19, especially if you experience any of the following symptoms:
    fever/headache/sore throat/cough/diarrhea/vomiting/fatigue/ muscle pain and/or joint problems/runny nose/shortness of breath

  6. Travel History - list of countries visited within the last 14 days

After completing the application form with all the required details listed above, carefully check the information provided to ensure that it is free of errors or typos.
Then submit the completed form and pay the service fee using any preferred online payment method.

What happens after applying

After submitting the completed Sri Lanka Health Declaration form, each traveler will receive via email a PDF confirmation of completion of the required declaration along with their unique QR code.
Make sure to provide a correct and working email address when completing the Sri Lanka Health Declaration so you can receive confirmation in PDF and QR code without any hassle.

Covid-19 entry requirements in Sri Lanka

Sri Lanka, like most countries, has relaxed entry requirements that were previously implemented.
As of March 2022, fully vaccinated travelers are no longer required to take a Covid-19 test before departure. Additionally, there is no need for quarantine or testing.
Children under 12 years old are exempt from pre-departure and post-arrival testing.
However, some restrictions still apply, such as travel bio-bubbles, reserved for travelers who are not fully vaccinated with specific hotels and attractions where they can used during stay.
People who are not fully vaccinated must remain in the travel bio-bubble for at least the first 7 days after arrival or until 14 days after completing the last dose of vaccination.
Additionally, travelers without proof of vaccination must have a PCR test within 72 hours of expected arrival or an antigen test within 48 hours of arrival.
Unvaccinated travelers must stay in the designated area for a full 14 days after arrival.
The area designated by the Sri Lankan government, known as the Tourism Bio-Bubble, offers hotels, tourist attractions and other amenities.
Another requirement maintained is that all travelers must have travel insurance that covers Covid-19, with a minimum coverage of $75,000 per month.
